Capturing crucial data
Our online Patient Questionnaires help practices monitor and capture crucial data on the health of their patients.
Questionnaires also help your practice monitor patients’ long-term conditions, and help you keep medical records updated. The process is simple; the patient receives a text message containing a link. The link then opens a secure online questionnaire – which can be accessed via a smartphone or an internet browser. After completing the questionnaire, the patient’s answers are recorded on their medical record using clinical codes. Their answers are also sent to the practice for review via the Mjog by Livi platform.
Securing QOF funding
Questionnaires also help practices secure Quality and Outcomes Framework (QOF) funding. QOF is the voluntary annual reward and incentive programme for all GP practices in England, detailing practice achievement results.
The system is designed to remunerate general practices for providing good quality care to their patients, and our questionnaires help practices better understand the requirements of the multiple groups of patients they treat; helping to improve the quality of health care delivered by the practice overall.
