According to Mind, one in four people will experience a mental health problem of some kind each year in England, and one in six people report experiencing a common mental health problem, like anxiety and depression, in any given week. The overall number of people reporting mental health problems has been going up in recent years.
Seasonal affective disorder (SAD), depression and anxiety become more of a cause for concern in the winter months, so we’ve recently been working in partnership with practices to create patient questionnaires on depression and anxiety, to help our GP partners monitor and capture crucial data on the mental health of their patients, remotely.
The questionnaire asks patients to rate how they are feeling on a scale, often giving a score – see example below.
The questionnaires then finish with some suggested next steps, depending on the patient’s answers, as shown below.
Completing ongoing review templates can be time consuming and make appointments disjointed, so we’ve designed an efficient solution to support the management of long term conditions.
Our digital questionnaires have been designed to help your practice collect structured data from patients, and to monitor long-term conditions. They can be completed in three easy steps:
- The patient receives a text message containing a link.
- The link then opens a secure online questionnaire.
- After completing the questionnaire, the patient’s answers are recorded on their medical record using clinical codes. Their answers are also sent to your Mjog inbox for review.
